Outer material is very soft and smooth. Pillow itself is very soft too! The feeling reminds me of a memory foam pillow -and thus it's a little heavier and more solid compared to any standard plush toys. I would go for a medium size if you were looking for a smaller cuddle buddy that would fit nice and snug into your arms. However if you are wanting something a bit more substantial to cuddle and have the space, then definitely get the large!
(Also I would agree with an earlier review that the pug has a cuter face IRL. But I love pugs, so take that with a grain of salt :P)
