Purpose of Use

Tokyo Otaku Mode Co. Ltd. (the "Company", “We”, “us”, “our") will acquire personal information by legal and fair means and shall use within the scope of the "Purposes of Use" as shown in the table below.If any other purpose arises which does not fall under the scope of the "Purposes of Use," the Company will (1) obtain consent from the person before using the information, if such information was directly from the subject person in writing, or (2) publicly announce the purpose of use , if the information is obtained by other means.

Sharing Personal Information with Third Parties

Other than providing personal information to third parties under the following conditions, we will not provide personal information to third parties without the consent of the individual, except in the following cases (a) through (c).

For those who use credit card payment

Purpose: To detect and prevent fraudulent use by credit card issuers.
Items provided: Name, e-mail address, billing address, shipping address, telephone number, information related to Internet usage environment, and other information related to purchases made on our website.
Method of provision: Provided directly including records made by electronic, magnetic, or other means unrecognizable to human perception
Recipient: The card issuer used by the customer.
Contract: In accordance with the terms and conditions of the card issuer being used.

Transfer of personal information to a third party in a foreign country

All such third parties to whom we provide your personal information in cases where they are located outside of Japan as described in (1) through (4) above have taken measures to protect your personal information that correspond to the eight principles of the OECD Privacy Guidelines.

Outsourcing of Personal Information

When outsourcing the handling of personal information, the Company will select a service provider that it deems meets its standards for personal information management and will oversee said company to ensure that personal information is handled appropriately.

Voluntary Nature of Providing Personal Information and Consequences of Not Providing Such Information

All items of personal information obtained will be provided by the individual concerned. However, failure to provide necessary information may hinder the procedures and processes outlined in the Purpose of Use.

Acquisition of Personal Information by Methods Not Easily Recognizable by the Individual

This form uses cookies for session management.

Requests for disclosure of personal information, etc.

  1. Upon request to notify Company’s purpose of use, disclosure, correction, addition, deletion, cease of use, deletion, or non-provision to a third party (collectively hereinafter, the "Disclosures") regarding a personal information from the pesron in subject, Company will conduct an internal investigation and respond to scuh person without delay.
    * However, depending on the content of the request, Company may not be able to correspond to the request for Disclosures. In such cases, Company will notify the reason for such non-correspondance.
  2. To make a request for Disclosures, please follow the procedure hereinafter provided and send your request by mail. Please note that we may not be able to accept your request if it is in incomplete or insufficient form. For each request for Disclosures of personal information or notification of purpose of use, an administration fee of 1,000 Japanese yen will be charged in the form of a prepaid postage.
  3. Personal information provided on the "Personal Information Disclosure Request Form" will be used to respond to inquiries and will not be used for any other purpose.

For request made by a person itself

To make a request for Disclosures, please prepare the following documents to be sent by delivery recorded mail.

    Please download the above file and fill in the required information.
  2. Identification documents
    A copy of a document that verifies the person's name and current address, such as a driver's license, passport, or health insurance card.

When user's representative makes a request

If a legal representative or an authorized representative is making a request for Disclosures, please enclose the following documents in addition to the documents listed in the preceding section hereinabove.

In the case of a legal representative

  1. Documents to confirm that the applicant has the right to represent legally.
    A copy of the family register or, in the case of a person with parental authority, a copy of the health insurance card with dependents' names entered is acceptable.
  2. Documents to verify the identity of the legal representative
    A copy of a document such as a driver's license, passport, or health insurance card that verifies the name and current address of the proxy.

In the case of a proxy by power of attorney

  1. Letter of attorney (document attached to the Personal Information Disclosure Request Form)
    Please download the above file and fill in the required information.
  2. Certificate of seal impression of the person requesting the Disclosures (issued within the last 3 months)
  3. Documents to verify the identity of the proxy by power of attorney
    A copy of a document that verifies the name and current address of the proxy, such as a driver's license, passport, or health insurance card.
